He reached a certain emotional/maturity age and then just stopped 'growing', for whatever reason in a serious case of arrested development.  Now has reached the age where he could/should have done something way back to get some help, but no doubt accused others of having the symptoms/issues he exhibits when any of them said anything to him about it, further entrenching his 'methods'.  Friends drifted away or hold him at arms length, except for maybe one or two who may have drifted into his orbit due to some common themes.  Family is dismayed, but see no way at this late date that anything can help...unless by some epiphany, he asks for it.   We see another significant case of this on the national stage right now, and that may be why he relates so well to all things Trumpy....he sees a Fellow Traveler and responds.   I bet any who know him see it.
